How It Works.

1. Individualized
Our outpatient PT, OT and Speech therapists can help you maintain or increase independence in your daily activities. Receiving therapy services in  your home is not only convenient but it allows our expert therapists to design interventions specific to you, your home environment and your support systems in order to maximize your health.

2. Separate from Home Health
Because outpatient services are separate from your home health benefits, you do not need to be homebound or have nursing needs to receive therapy in your home.

3. No Referral Necessary
We can get you started on outpatient therapy with or without a physician order. We will still send updates to your physician on your progress.

4. Get Started
With some basic information, we will tell you if there is a copay for your outpatient therapy or if your insurance(s) will cover the entire cost. Then we will match a therapist to your goals who will come to your home to get started!

I was planning to attend a clinic for my therapy, why should I choose Outpatient at Home?

For many patients who require outpatient therapy, getting to a clinic is no problem. These patients should certainly attend an outpatient clinic. The patients who will benefit from outpatient services at home are those who cannot or do not want to go to a clinic. Their options are either stop therapy altogether or have a therapist come to their home. This is a gap in care we’ve identified and with recent infection concerns related to COVID-19, the gap is getting ever larger.

An outpatient clinic has large equipment, how can you provide the same level of therapy as a clinic in my home?

We understand that there are some things we cannot provide when coming into your home. When we do the evaluation, if our therapist believes you have needs that we cannot meet, they will get you set up with a clinic who can. However, we do have equipment such as modalities for pain relief and muscle stimulation that are portable and able to be brought into your home and our skilled therapists are very talented at using creative ways to meet your therapy needs with the equipment they/you have available.

Do I need to be homebound or have any formal reason I can’t get to a clinic?

No. Outpatient services can be provided in your home for any reason. You do not have to be considered homebound or have nursing needs, nor do you need any formal reason that you cannot travel to a clinic. Reasons we hear often include transportation issues, not wanting to go out in the cold, COVID-19 infection concerns, wanting one on one therapy, or keeping the same therapists they’re already familiar with.

Can I stay on with my home health nurse while I’m getting Outpatient at Home therapy?

Unfortunately Medicare does not allow both home health and outpatient services to be billed at the same time, so even though we’d be seeing you in your home, our services are considered outpatient and cannot be billed while you’re still active in a home health episode. Once home health services discharge, we can begin seeing you for outpatient therapy. Outpatient at Home does not include private pay nursing or aide services, but our Wellness at Home program does.

I was working with a great therapist for home health, can I keep them under Outpatient at Home?

If you were receiving therapy from our home health therapists and now qualify for Outpatient at Home, we can keep the same therapists you were working with as long as it works with their schedules. This is one of the attractive features of our program because finding a team who you’re comfortable with and who supports you through your rehab journey is so important in your success! The ability to keep that team even as you switch programs is crucial.

If circumstances change and I decide I want to go to a clinic, will I be allowed to do that?

Yes! We would be happy to help you transition to a clinic of your choice and will share your records with them ensuing a smooth transition.

Do I need a doctor’s referral?

Our therapists have direct access, which means they can evaluate you and set up a plan of care without a doctor’s order. If you are referred from a source other than your doctor, we can get things up and running without waiting for the doctor’s order. We do send the documentation to your doctor, but we do not need an order to initiate services.

Will my insurance cover this?

Medicare typically covers 80% and then if you have a secondary insurance, many times they will cover the remaining 20%. We accept Medicare and most private insurances. Normal private insurance co-pays may apply. The best way to know for sure is to call and allow us to complete a complimentary, no commitment, evaluation of your benefits.
